Fill out this form to report a lost or request a uniform item. Your instructor/ S4 will be notified and can help you locate or replace the item.

Important Information

Some items may not be available in the JROTC supply room. Check with your instructor or S4 before ordering replacements. Some items may require a replacement fee.

What Happens Next?
  1. 1.Your instructor/S4 will receive your lost/Request item report
  2. 2.They will check the availibility of item in the JROTC supply room.
  3. 3.You'll be contacted about if the item isn't available.
  4. 4.Some items may require a replacement fee - check with your S4.
